Advance Care Planning Assistance

Advance care planning allows people to have a say in what level of health care they will receive if they become unable to speak for themselves. An advance care plan tells others what is important when it comes to health interventions and may name another person to follow through with their wishes or make decisions for them.

 

While everything you need to complete an advanced care plan is available online at http://www.advancecareplanning.ca, you may not have the time or the depth of understanding needed to complete an advance health directive, or the skills needed to facilitate a family discussion about directives for yourself, another family member, or friend. The in-depth self-assessment and/or the sometimes difficult conversations that are necessary to get an advanced care plan in place can lead people to put it off, often until it is too late.
